At Bradley Senior Success, our services are individualized to the Senior, and modified when needed. Every effort is made to allow the Senior to STAY AT HOME for as long as possible to live independently and with dignity. If it is necessary, due to safety concerns, to place the Senior in a nursing home or other facility a Bradley RN Care Manager can continue to provide services to assure quality of care.

Having a Bradley RN Care Manager review and monitor the Senior’s medical care and medications will ensure a healthier, more worry-free, and oftentimes a longer life for Seniors.

A Bradley Nurse Care Manager becomes acquainted and builds a rapport with the Senior. We then proceed with an Initial Assessment. The information gathered from the assessment serves as a tool for developing a Care Plan. This Care Plan is presented to the family members who then decide which interventions they wish to have completed. We do as little or as much as you wish.

- Personal Emergency Response System (call tag)
- Encourage outside activities and social networks (i.e. Church functions, Senior Citizen Centers)

Financial Planning:
- Bill paying
- Filing medical insurance claims
- Assist with any necessary forms or paperwork for taxes

Case Management:
- Comprehensive health assessments
- Recommend Healthcare Professionals and Provide Quality Assurance
- Healthcare Utilization Review (Coordinate care between specialists i.e. cardiologist and internist)
- Drug Utilization Review (Prescription and OTC drugs are reviewed for any possible adverse interactions.)
- Provide a personalized plan of care for immediate and long-term services
- Implement plan of care as chosen by the Senior/Family.
- Keep Family Members informed of Medical Concerns/Issues

Crisis/Transition Management:
- 24/7 access to a Bradley RN Care Manager
