Our online referral process is a safe, simple, efficient, and quick way to obtain Bradley Consulting and Management services. Clicking the “New Referral” button below starts the process. During each step, simply complete all available information and click “Continue” to proceed. There is no need to fill in everything, but your contact information is very important. The information will be sent directly to BCM.

The BCM office will review the information provided and match a nurse to the case. The case manager will phone the referring adjuster the same day to confirm and discuss details of the assignment.

This online referral process has been designed to make it easier for you to provide information to BCM. If you prefer, you may print the referral sheet and complete it by hand then fax it to BCM at 317-733-9407.
